Your co-payment and/or co-insurance must be paid at each visit. We will file your insurance if we are contracted with your insurance company. Any balance unpaid by your insurance is your responsibility.
Any disputes about coverage or benefits are your responsibility and are between you and your insurance carrier. If you have questions regarding coverage/payment, you must direct those inquiries to your insurance carrier. Remember to always get a reference number and the name of the person you spoke with.
